Updates to the Assistive Devices Program’s (ADP) Processes for Receiving Applications for ADP-funded Devices and Supplies Grants

The ADP is Accepting New Applications for ADP-funded Devices by Fax:

On April 29, 2020, the Assistive Devices Program (ADP) requested registered vendors and authorizers to commence submitting new applications for ADP-funded devices by fax to 1-888-222-8018. Applications submitted by fax must be complete (forms properly filled out, signed, and all pages included). ADP-registered vendors are required to retain the original applications for faxed applications.

Any corrections and/or additional information respecting existing or new applications must also be submitted to the same fax number. Any applications which were already mailed to the ADP office will be processed and unless returned to the sender, should not be resubmitted by fax. All questions respecting this new temporary process for submitting ADP-funded device applications should be sent directly to the ADP: [email protected].

The ADP is Accepting New Applications for Supplies Grants via Email:

The ADP also updated its processes to allow eligible Ontarians to apply for ADP-funded supplies via email. Applicants can now print and complete the application form, with the help of their health care provider(s) if applicable, and send a copy via email to: [email protected] 

Applicants must keep their original application but are not required to mail it to the ADP office until further notice. Applicants who previously applied to the ADP for supplies funding by mail are not required to resubmit their application via email. The ADP continues to process past applications received by mail.
