Anyone can file a complaint about an occupational therapist with the College. All complaints received by the College are addressed.
Please use our online form to share your concerns or submit your complaint.
You must provide:
- the name of the occupational therapist you are concerned about,
- details about the nature of your concerns, and
- your full contact information.
If you are unsure of the occupational therapist’s name, please contact us and we will help you.

If you do not receive an email confirmation, please contact the College. The College will also notify the occupational therapist to your complaint within 14 days of the College filing your complaint.
