You must provide the name of the OT you are concerned about, as much detail as possible about the nature of your concerns, as well as your full contact information. If you are unsure of the OT's name, please contact us and we will help you.

Anyone can file a complaint with the College. All complaints received by the College are investigated. Learn more about the process at Report Concerns or File a Complaint.

After you submit the online complaint form, you should receive a confirmation email from the College within 2 business days. If you do not receive an email confirmation, please contact the College. The College will also notify the OT to your complaint within 14 days of the College filing your complaint.
