- the name of the occupational therapist you are concerned about,
- as much detail as possible about the nature of your concerns, and
- your full contact information.
If you are unsure of the occupational therapist's name, please contact us and we will help you.
Anyone can file a complaint with the College. All complaints received by the College are investigated. Learn more about the process at How to Report a Concern or File a Complaint.
If you would like further information about making a complaint or if you are unsure if you would like to file a formal complaint, please contact the College at 416.214.1177/1.800.890.6570 x234 or investigations@coto.org.
After you submit the online complaint form, you should receive a confirmation email from the College within 2 business days.
If you do not receive an email confirmation, please contact the College. The College will also notify the occupational therapist to your complaint within 14 days of the College filing your complaint.
Respectful behaviour
We are committed to communicating with you in a respectful, professional and civil manner. We expect the same courtesy from you.
We understand that your circumstances may be stressful and emotional. However, we will not accept abusive behaviour towards our staff.
For safety purposes, threatening or abusive comments to staff will not be tolerated. Staff reserve the right to end communication right away in these situations. Repeated abusive conduct may result in the COTO communicating with you only in writing or otherwise restricting future communications with you.
